New Client Paperwork & Your First Visit

Please print and complete the Registration and Intake Form

Bring the completed form(s) with you for your first visit.

If you are bringing a minor child, please print the Child Intake Form and have the child complete the form to the best of their abilities.

 If you have any questions about the paperwork, please leave that portion blank, and you can talk with your therapist during your first visit.

We have also included a document that outlines a Client's Bill of Rights. We hope you find this helpful and informative.

Please plan to arrive 10 minutes prior to your scheduled appointment. You will find a clipboard in the waiting area where you will complete the remaining paperwork. When you are finished, your therapist will meet you in the waiting area.
